read moreMemorial folders come in packages of 100 folders and are custom printed with a poem or verse of your choice and information about your loved one on the inside. You may choose from our poem book or provide your own poem or verse. We can also make custom folders with a picture of your loved one on the front or inside. Memorial Folders are $95.00 for the first 100 folders and $75.00 for each additional 100 folders printed with the same order. Add a custom picture for $50.00. Boxed Sets featuring artwork by Thomas Kinkade include a Guest Book, 100 Memorial Folders, 2 boxes of 25 Acknowledgement Cards with envelopes, and miscellaneous items.

read moreFor burial you will need forms 1-4,6-7. For cremation you will need forms 1-7. Form 2 (Hospital Release) is needed only if the decedent passed away (or may pass away) in a hospital. Form 3 (Preneed Disclosure) is needed only after the decedent has passed away. Other forms may be needed for special situations.
