The Case Manager will call us with permission to complete an evaluation. Your clinical information will be reviewed by our admissions committee to determine how best to meet your needs. If accepted, the attending physician at the hospital will approve your discharge from the hospital and write your admission orders to Denton Rehabilitation & Nursing Center. Please let the Social Worker at the other facility know that we have permission to see your medical chart.
