read moreFounded in 2013, Altair ACO began with six HCBS providers (Lutheran Social Service of MN, Hammer Residences, Living Well Disabilities, Lifeworks, Opportunity Partners, and Mount Olivet Rolling Acres) coming together to share best practices for supporting quality lives of individuals with disabilities. The collaborative grew throughout the years, focusing our mission and adding members in 2016, 2018, 2019 and again in 2020. Today, our disability led social Accountable Care organization is a collaboration of 14 providers supporting over 20,000 individuals statewide in Minnesota, and more than 30% of the disability waiver population.
