Diabetes Education
read moreWhether you are hospitalized for a diabetic condition or another ailment, your diabetic team will work with your physician while you are an inpatient at Jackson Park Hospital. If you do not have a primary care physician, you may choose one of our Board Certified physicians. When appropriate, your doctor may refer you to a specialist or other physicians. You can fax your blood sugar levels, diet, and exercise results to your Diabetes Educator. Also, you can call to schedule classes, to ask questions about your diabetes care or to get more information about our program and services.
Partial Hospitalization Program
read moreThe Jackson Park Hospital Partial Hospitalization Program (PHP) provides treatment to individuals with acute and persistent psychiatric disorders who require coordinated, intensive and comprehensive multidisciplinary treatment. The hospital's PHP serves as an option for individuals to continue an intensive level of care after an inpatient hospitalization. PHP serves as an alternative to inpatient hospitalization when an individual is experiencing acute symptoms of mental illness, but does not require 24 hour care and in instances when outpatient care has been determined to be insufficient and/or ineffective.

Hemodialysis
read moreJPH Dialysis Center was opened in 1978 in the basement of the hospital with 20 stations. We moved to the first floor in November 2005 and increased our stations to 23. We have the capacity to dialyze 23 patients each shift for three shifts a day. We are very pleased with the state of the art equipment provided by Fresenius Medical Care. Patients at our center are receiving the best medical care comparable to the best anywhere in the country.
