In the absence of a physician referral, physical therapy services can be offered for thirty days after the initial physical therapy evaluation. Thereafter, the patient must be referred to the physician and a referral as detailed above is required for continued physical therapy services. The customer should verify with the insurance carrier that physical therapy services are covered and be pre-approved without a physician's referral.
