ACT supplies services directly onsite or via Telehealth (face-to-face through the computer) to the residents of assisted living facilities, skilled/nursing facilities and rest homes. By serving individuals in their familiar setting, ACT clinicians are able to completely assess presenting problems and to incorporate active caregivers into plans for improving the quality of life for patients. Our preventative care, care oversight, medication management and various forms of therapy are all conducted with the mission of delivering the most efficient and least intrusive care available to the individuals we serve.
