You'll upload your information including insurance information (if you do not have insurance and want coverage with the CARES plan - you need to upload your SSN# to verify you are in the database). When you come in for your appointment at our Lakeview location, you will have a brief telemedicine visit from your car (in-person is also permitted for those on foot - our heated garage is prepared to accompany you). We will ask you your symptoms and procure your tests (this is when you can request a rapid test if you desire).
