Dr. Anjali Modi is a board certified pediatrician and fellow of the American Academy of Pediatrics. At a very young age she started apprenticing under her physician parents, and knew that medicine was her calling. She graduated at the top of her class in medical school and did her residencyat Inova Fairfax Hospital in Northern Virginia. She has been practicing pediatricsfor 11 years. She has recently relocated back to the DC area, after working for a few years in Atlanta and Chicago.

Health forms are required by many organizations including schools, daycare, camps, and school athletic departments. Health forms that are brought to a well child examination will be completed at that office visit. When forms are required at other times, they will be completed within 7-10 business days. There is a $15 ( as of Jan 11, 2021) charge for each form.
