read moreUpon calling to schedule an initial appointment, patients will be required to fill out and return new patient paperwork. Completed paperwork can be faxed to 614.273.0173. Once this paperwork is received and reviewed by our consulting physician, the initial appointment will be scheduled. Patients must bring their insurance card and physician referral, if applicable, to their first appointment. Patients are encouraged to dress in comfortable clothes that allow for movement as our treatments are interactive and require patient participation.
