Our main mission is to prevent anyone being admitted to institutional care until it is the last resort. Starting out as a Medicaid Personal Care Aide program, over the years we have expanded our services to include Advantage Waiver, Medically Fragile, Veteran's Program, Private Insurance, and Private Pay.

ADvantage Case Management Provides assistance for consumers from a certified case manager in gaining access to needed waiver and other State plan services, as well as, needed medical, social, educational and other services, regardless of the funding source for the services to which access is gained.
